Dear MPTCS Parents:
 
Our 2016-2017 school year is upon us.  As we begin the year it is imperative that we receive information from you to make sure your child can successfully start school. At the end of the school year and throughout the summer we have mailed forms to you for your completion.  The submission of these documents impacts our funding and scholar programs, therefore it is of the utmost importance that we receive all documents from you as soon as possible to avoid the disruption of your child's attendance.  We will need the following documents:
 
  • Address verification documents (3 proofs of residency, including utility bills issues after March 1, 2016; driver's license and state-issued documents featuring your home address).
  • Lunch applications (must be completed by all families whether you qualify, or not, for free or reduced lunch)
  • Health form (all scholars must have an annual physical and health form on file)
  • Media Release form
  • Language Survey
  • Social history form
